Photo Release Form

From time to time, pictures and videos may be taken of parish activities and celebrations. Our parish Pax Christi in Lexington, Ky would like to be able to use these photographs and videos for promotional and educational material like flyers, brochures, social media and on the parish’s website. Written consent of both the child’s parent/guardian, if applicable, is required. I authorize and give full consent, without limitation or reservation, to this named parish: Pax Christi to publish any photographs or videos of my child at parish-related activities and celebration. Images may be published in future publications, brochures, flyers, educational materials, webpage, social media, or other promotional materials. There will be no compensation for use of any photograph or video at the time of publication or in the future. I waive the right to approve the final product. I agree that all such portraits, pictures, photographs, video and audio recordings, and any reproductions thereof, and all negatives, recording tape and digital files are and shall remain the property of Pax Christi parish. I understand and agree that the parish may use these images in subsequent years unless I revoke this authorization by notifying Pax Christi Parish in writing. If there are concerns about pictures or videos posted on the website, please contact the parish office and they will promptly be removed. I authorize Pax Christi Parish to publish the first name of my child with any photograph or video in which my child appears. I hereby release, acquit and forever discharge Pax Christi Parish and the Diocese of Lexington, its current and former officers and employees of the above-named entity from any and all claims, demands, rights, promises, damages and liabilities arising out of or in connection with the use or distribution of said photographs and/or video recordings, including but not limited to any claims for invasion of privacy, appropriation of likeness or defamation.
